Hole in the Head (HITH / HLLE)

severe severityparasiticfreshwater21-day treatment

HITH/HLLE creates visible pits and erosions on the head and lateral line. Most common in cichlids (oscars, discus, angelfish). Caused by Hexamita parasite exacerbated by poor nutrition and water quality.

Symptoms to Watch For

  • โ—Small pits or holes forming on head and lateral line
  • โ—White stringy mucus from lesions
  • โ—Loss of color around affected areas
  • โ—Weight loss despite eating

Common Causes

  • โ–ธHexamita parasite (flagellate protozoan)
  • โ–ธPoor water quality and high nitrates
  • โ–ธNutritional deficiency (vitamin C, vitamin D)
  • โ–ธActivated carbon use depleting trace minerals
  • โ–ธStress from overcrowding

Treatment Protocol for Hole in the Head (HITH / HLLE)

Treat with metronidazole (Flagyl) โ€” 250mg per 10 gallons, repeat every 48 hours for 3 treatments with 25% water changes between doses. Improve diet with vitamin-enriched foods. Remove activated carbon. Lower nitrates below 20 ppm. Often seen in cichlids, discus, and oscars.

How to Identify This Condition

Look for these symptoms in your fish:

  • Small pits or holes forming on head and lateral line
  • White stringy mucus from lesions
  • Loss of color around affected areas
  • Weight loss despite eating

Early detection is critical โ€” the sooner you begin treatment, the better the prognosis. Observe your fish daily during routine feeding for any behavioral or physical changes.

Root Causes

Understanding what caused the problem helps you prevent recurrence:

  • Hexamita parasite (flagellate protozoan)
  • Poor water quality and high nitrates
  • Nutritional deficiency (vitamin C, vitamin D)
  • Activated carbon use depleting trace minerals
  • Stress from overcrowding

Recommended Medications

The following medications are effective against Hole in the Head (HITH / HLLE):

  • Metronidazole โ€” follow manufacturer dosing instructions. Treat in a hospital/quarantine tank when possible to avoid disrupting your main tank's biological filtration.
  • Seachem MetroPlex โ€” follow manufacturer dosing instructions. Treat in a hospital/quarantine tank when possible to avoid disrupting your main tank's biological filtration.
  • Vitamin-enriched food โ€” follow manufacturer dosing instructions. Treat in a hospital/quarantine tank when possible to avoid disrupting your main tank's biological filtration.

Always have these medications on hand before you need them. See our quarantine tank guide for proper treatment setup.

Water Quality During Treatment

Maintain pristine water conditions during treatment. Perform 25% daily water changes using Seachem Prime as your water conditioner. Remove activated carbon from your filter during medication (carbon absorbs medications). Increase aeration with an air stone โ€” sick fish need extra oxygen. Monitor ammonia and nitrite daily with your test kit.

Recovery & Quarantine

This is a severe condition that requires prompt treatment. Isolate affected fish in a hospital tank if possible. Continue treatment for the full 21-day course. Watch remaining tank mates closely for early signs of infection.

Prevention

The best treatment is prevention. Follow these practices to reduce the risk:

  • Keep nitrates below 20 ppm
  • Feed varied, vitamin-rich diet
  • Avoid prolonged activated carbon use
  • Maintain proper mineral content in water

For a complete prevention strategy, read our quarantine tank guide and maintenance schedule.
